connecting form submission to SSL

I am new to SSL and while I have the SSL set up with the host, I'm not sure it's doing what it's supposed to. The site is http://www.northwestneurology.net and one of the pages in question is http://www.northwestneurology.net/patient-health-form/. We have the SSL set up with the host. I've set the URLs to https:// in WP. In addition, I'm running these forms on Machforms, installed behind the WP site and pulled into WP with js. I've set the Machforms to also be https:// but I'm not really sure what is supposed to happen. We don't care too much about the rest of the site, but we want these forms to be submitted via the SSL. Any direction would be greatly appreciated. Thanks!

    I've checked your form page:
    https://www.northwestneurology.net/patient-health-form/

and I think everything is correct already. I can see that your form is linked using https and the browser is displaying the padlock icon on the address bar (which mean you have a secured page).

So you should be good with that. The form should be submitted via SSL already.
