   I followed the instructions from the following post:

   The short code works fine, but after the page is loaded IE (7 & 8) goes to 404. It works fine in Firefox, Chrome and Safari.

   Ok... After some investigation I realized that the date field is causing the page to crash (I'm not sure why). I setup a test page which is identical to the page I am having the issue, minus the date field, and it works. Here is the link: http://reunion.furtados.net/form-test/

   I must say this is really weird. The only difference when using the date field is that your form would load "calendar.js".

   I'm not sure why it doesn't seems to work on that page, but it might having some conflict with the existing javascript.

   To check this further, is it possible for me to have temporary FTP login to your machform?
