What can it do??

    SO this script uses AJAX to generate the forms to display and then allows the collection of input data directly into its database???

    Does it have any templates to show that data to the web (rather than the editable forms in the ACP

    Yes, MachForm allows you to create form and collect data to your database and email.
    It doesn't have the template you are looking for though.

    Or do you mean form themes?
