Same form displayed on multiple pages

I have added a contact us form to our website. This contact us form is displayed on the footer. I also have other forms on different pages. There is a conflict with displaying multiple forms on the same page with the php code. All other forms change to what's displayed in the footer. When I change the footer to include the iFrame code the other pagers display correctly. Is there a way I can use the php code in the footer as well? (The php code adapts a little better to responsive pages as there is no fixed height like there is with the iFrame code, even with the scroll bar turned on). I think it has something to do with mf_placeholder.

I'm afraid you can't embed multiple forms within a single page using php code or javascript code. Sorry. You'll need to use combination of javascript + iframe or php + javascript.

Hello, I just found this post after reading two other threads. I thought it may be helpful if you posted a link to this page on these two threads for future readers. I didn't find the answer you gave above there. Thanks.
