   This just started happening and I can't seem to figure out why, I get the below error when I try to submit my form.

   INSERT INTOap_form_1(element_1,element_2,element_3,element_4,element_6,element_7,element_8,element_9,element_10,element_11,element_12,element_13,element_14_1,ip_address,date_created) VALUES ('test3','30','10','8','0','0','0','5','0','0','0','0','1','75.160.74.53','2009-07-21 09:57:37'); Query failed: Duplicate entry '' for key 1

   Anyone know what this means?

   This seems to be an error with your form table. Do you have many entries already for that form?

   If not, try to delete all entries for that form.
   If you already have important entries, we'll need to check into the table directly.

   Send us your FTP login info so we can check it directly.
