Problem embedding in Wordpress php template

Love the product, but I'm having problems embedding advanced form code in a Wordpress template. To be clear, I am inserting the code directly into the Wordpress php file, not via the admin write panel. I've edited out my table name.
Here is the result:
select form_name, form_description, form_redirect, form_success_message, form_password, form_unique_ip, form_frame_height, form_has_css, form_active, form_captcha, form_review from ap_forms where form_id='1' Query failed: Table 'edited_wordpress.ap_forms' doesn't exist

Hmm.. without checking your file directly, it would be difficult to trace it.

Any resolution to this?
I'm having the same issue when embedding the advanced form code in a WordPress page.
The query fails to find "ap_forms" as the mysql_connect is going to the WordPress database instead of the Machform database.

I need help with this too!! I'll definitely be buying more licenses if I can get this to work with wordpress. I'm getting the same error as above... and I am actually using the integration code from the post you recommended above.
My error looks like this:

I don't know if it's a wordpress issue, or a machform issue.... but like the others have said, machform is looking at the wordpress db instead of the forms db.
