php require of "users-functions.php" missing from add_widget.php

    As mentioned in the post title, it appears that the following require:

    require('includes/users-functions.php');

    is missing from the "add_widget.php" file in a fresh install of Machforms V4. Just FYI Yuniar and co!

    FYI it appears this require is also missing from report.php.
