php form file not working

hi,
i've generated the PHP form file in the form code section & it has the name booking.php. i've uploaded it into the same directory where machform is installed. when i then try to call <machform-web-url>/booking.php i get an empy page. what could be the reason?

I'm not sure why (or if?) you created your form using phpform (http://www.phpform.org) but if so, it wasn't necessary.
You should be using Machform to create your forms which also has the full capability to manage those forms, and entries submitted as well as many other features.
The only thing you need to do to get a form to be in a web page is the code-snippet that is generated when you create and save a form into your web page where you want that form to appear. (The code associated with the specific form is available under the "code" button in the form manager list)

Please forgive me if you already know the above, but from your question it is apparent to me you appear to be confused over how Machform works.

i've not used phpform, but machform ("code" button in the form manager list) to generate the form code:
https://dl.dropboxusercontent.com/u/2216927/form_code.jpgi've then downloaded the file ("booking.php", because the name of my machform is "booking") & placed it in the same directory where machform is installed. when i open <machform-web-url>/booking.php in my browser, i get an empty page.
