PHP Errors with Multi-Page Form

    Hi -

    I also sent this to support, but wondering if anyone on the forums could help. I'm using v.3 to create a multi-page form. I got these PHP errors (see below) when I started entering the page breaks and now the form is not working. Looks ok in edit mode, but when I "View" it, only shows 2 of 6 pages and fields are not present, just the text for them.

    Any advice?
