Multiple Text/Number fields on same line

I have the need to create a questionnaire form with multiple fields that are related to each other. For example:
3. Age
Write the number of family members in each age bracket.
____ Birth–5 years ____ 30–39 years
____ 6–11 years ____ 40–49 years
____ 12–17 years ____ 50–59 years
____ 18–29 years ____ 60 or more yearsI love being able to put radio-buttons inline/same line but would like to do the same as multiple-choice but with number fields and text.
Suggestions?

Yeah, I saw that after I put in this post. I have used these but in some cases the alignment squeezes fields down to smaller than their original/defined size.
Speaking of, the forms seem to always size at around 700px wide. Is this value defined somewhere I am not seeing, so I can change it?
