    How do I make it go away?

    You can use the Theme Editor to create a new theme that hides the logo, clears out the background color. Save your theme, and then apply it to the form whose header you want to remove. If you want to remove the Title and Description as well then you'll have to edit the new theme css file that's created after you save your theme. The file should be located in: /machforminstall/data/themes/theme_#.css.
