javascript default values not showing up in form on last page

    I have been using the javascript method of calling up the machform. The default values all seem to show up fine but they are not working on the last page anymore (after I added 5 more fields today). I am up to element_275 now in my form so it is pretty big.

    Any idea why this would be happening?
