Issues with redirect and post after submission on PHP emebed

    I am able to successfully embed and receive results from the iframe version of my form, however, with I try to include the PHP code the form loads, allows me to fill it out but when I click submit it re-routes me to a page that doesn't exist (mydomain.com/#main_body), and not results are yielded on the back-end, any suggestions would be great, thanks in advance.
