Https page loses green padlock if machform code is inserted in wp blank page

Hello, I've been trying to resolve an https issue in chrome 46 for 6 hours, and cannot. Here it goes:
The page in wordpress is with https and works fine until I insert the machform code (also https!). Before insertion, google chrome (version 46) displays the green padlock. After insertion, there is no padlock at all (in chrome 46 this is the new way to show "mixed content" not trusted).
What should I do to get both machform in the page and the green padlock?Steps I have taken
switched machform from http to https to get https codes (I have MachForm Version 4.5)
wordpress https plugin added to have exclusive https wordpress pages (not all site is https)
wordpress relative url image plugin added to have all images with https
I have added a blank page template to the wp theme used
the blank page gets the green padlock if I only add the "simple link" in the machform code (simulated test page): https://budapestrivercruise.com/a-la-carte-dinner-cruise-booking-with-card-2
the blank page loses the green padlock (it shows briefly and then goes away) the instant I add the javascript of the code. I have also tried the iframe code, doesn't work (no green padlock). I have tried to add the embed php code directly in the blank page template, just to see if it could work as a php page: the page shows but deserved no green padlock from chrome.
I also created a non blank page to see what elements make the secure site symbol lost. It is unfortunately machform. Page with site template (standard) without machform code beautifully shows green padlock: https://budapestrivercruise.com/a-la-carte-payment-testingSo at this moment we are using a page for payment that is not secure according to google chrome. It is not red alert, but not green padlock either. I think customers would trust us better if we had the green padlock displayed.
Whynopadlock shows the page is perfectly all right if the machform js code is embedded on the page, but not chrome.
Any ideas? Please help (note: I'm not a programmer, and do not understand programming lingo.)

In Firefox or Chrome I don't get any "mixed secure and non-secure elements" message because the form is not displaying in either case. Can you check the simple link and embed code again?

Hello AMurray, the above links do not have the embedded code, only show that the pages linked above have the green padlock _before_ the insertion of the code.
I have created a page where you can see an embedded form too:

Can confirm now what you're describing (I probably didn't notice it last time) - in Firefox - the padlock SSL is visible.
In Chrome, for one second the padlock comes up as the page with the form loads, then goes away. The icon when hovering the mouse over says "Show Site Information"
