Hide Entry number when printing?

    I am a Photographer and use MachForm (3.5) to have my clients reserve a date for their events online. It works great. However after they submit it, I then go to the entry via my control panel and print it as a PDF and email a copy to my customers. At the top of the PDF, it shows the form name, then a blue graphical arrow and then the entry #.

    Form name > #38

    This information is irrelevant to my customers. Is there a way to hide this from my PDF print outs?

    @lifesharephoto: If you know html and you are comfortable editing your Machform database directly (which can have disastrous consequences if you make a mistake), then it should be possible to edit the format of the default PDF generated by Machform to have any appearance you like:
