Help!!! Getting error messages

    Hi
    My client is getting the following error messages when she is trying to submit my form (contract) to me. How do I fix this? How do I fix this??? Please explain for "dummies"- I have no coding skills!

    Warning: Invalid argument supplied for foreach() in /home/shannonc/public_html/machform/includes/entry-functions.php on line 355

    Warning: Cannot modify header information - headers already sent by (output started at /home/shannonc/public_html/machform/includes/entry-functions.php:355) in /home/shannonc/public_html/machform/view.php on line 93

    What version of machform do you use? Also, it would be helpful if you could post the URL of your form. So that we could check it.
