Having three columns of different fields

    Is it possible to have one line across the form, as in three columns, but the first column being checkboxes, the second two columns text boxes? This way a box could be checked, and notes could be added to the side on that same line?

    Yes, it is possible to do so. Simply put "column_3" into the Custom CSS Class property of those fields.
