Having problems receiving 1 customers form

Every time we have a complaint about the form, it always comes from the same person saying that he did not receive his email confirmation. It is true that he didn't receive it because I did not receive it as well. But when I log onto the admin panel it is there and there does not seem to be a problem with his entry data. It doesn't seem to happen to anyone else.

Oh yea, by the way, I have deleted all entries in hopes that it may just be a server issue, but it does not resolve this problem.

Have you set the Email option (Machform Admin Control Panel) to send the email to the customer and to yourself?
Have you set the correct options in config.php so the script can send the email? i.e either sendmail or set the SMTP server options?
For the email to work (sending to the customer) the form must contain a field of type "Email" and it should really be a required field, since the person could easily overlook it.
Ask your host/provider if not sure about the sendmail or SMTP server details.

Yes, I've used the form successfully for quite some time with little problems. The email is required to complete the order, but for some reason, this one and only customer orders and the emails don't go through some of the time. I do log onto the admin panel and it is there but the email will not go through to him or me, sometimes until a day or two later.
