Form Styling into a table.

    Hi - I'd like to make a form field to look like this. Can you please give me some guidance on how I might achieve that?
    Here is a screenshot of how I would like this portion of the form to look http://www.bazoogle.com/1.png (except I want the prettier Machforms styling ;).
