Fatal Error when adding 4th form

    Hey everyone,

I'm finishing up on a project and I'm utilizing 4 total forms on the site. I'm using the advanced method of including it into the website but I'm having an issue. Whenever I add a 4th form to the website, no matter which form it is, I get the following error:

Fatal error: Cannot redeclare display_machform() (previously declared in /home/myusername/public_html/forms/machform.php:27) in /home/myusername/public_html/forms/machform.php on line 237

    Can anyone help?

Actually I know what the problem is now. You see on the sidebar I'm including one of the forms which shows on all pages. I have 3 other pages with forms as well, and if I visit any of those pages it gives me the error.

So the issue is that 2 machforms cannot be on the same page, is there anyway around this?

If you are using the advanced code, you can only embed one form for every page. Putting two forms would create conflicts indeed.

You need to use the standard form code instead, to embed multiple forms within one page.
