Are you sure its a valid email address that you're sending to?
In the emails "to You" field, you separate email addresses with a comma (,).
Can you try sending an email to petersenreporting the normal way (through an email client like Outlook) and see if she is receiving email of any sort?
She may also need to check that the email from Machform is not going to spam folder, or even being filtered (at her ISP mail server) before it gets to her.
What email or domain is specified as the "from" address for your form? She may need to mark your email address (by that, I mean the email from which Machform sends the emails as specified in the email options) as "friend" to remove it from blacklist or "junk" list if it's been marked inadvertently as spam.

The form I put together was easy, but now what? I tried to fill out the form and do a test but it doesn't know where to send it. How do I make it so that if someone fills out the form the info gets emailed to me. Thanks for your help and your program!!

Jason, in the form manager page you need to click on the form you are working on then click "emails" which is between to edit css and embed code. Once you are on the "Send notification emails to" page you can set all that up. **note** only make changes to one side at a time. (i.e. fill out the "You" side then hit save. Then do the "Your Users" side then hit save. or it won't save them both.)
The only confusing part about the email setup is the SMTP which you need to setup on your config.php file unless you are running the beta which Yuniar made it so you can make those changes in the graphical interface. Version 3 will make setup a lot easier!
