Crash and problems with resumed forms

My first adventures with a "resumed" form and I have major issues/crashes.
First off, the form is launched from my own customized pages. When the form is resumed, it pulls up the generic, un-customized pages you see when you "view" the form, not from the location it is originally launched from.
Second, the second page values are missing all the "default values" I put into every field. I had to figure some way for force folks to select at least 1-value for multiple entries so I made them all "required" and put zero/0 as the default value. This worked until I resumed the form. Then it fell to pieces. All the zeros became blank.
Third, even after I manually filled in zeros for the missing/required fields, submit crashes with:
