   Hi Yuniar,

   I have checkboxes at the bottom of this page

   http://www.chaiyil.co.za/machform/view.php?id=13

   When the form is submitted it is sent to specified address.

   The recipient doesn't know what was the QUESTION at the checkboxes, it only displays the selections, YES or NO.

   hope it makes sense.

   Hi Regardt,

   For checkboxes -- MachForm only sent the selections not the question. Since checkboxes are usually using descriptive labels.

   In your case (a simple yes/no question), I suggest to use Multiple Choice (radio buttons) instead of checkboxes. That way you will get the Question label too in the email.

   Also, using radio buttons prevent your users from choosing two answers all together.
