Certain forms aren't saving

I have a user that is creating forms and once she saves it once, it can't be saved again. It goes through the whole process of saving, but it never does. What could be the problem with this? Her browser? Her user account? Once she gets a hold of a form and saves it, it is corrupted for every user, not just her.

With the help of yuniar, it was incorrect closing of html tags that was causing this error. So for future searchers, make sure all your html is proper first.
