Can Send Email Notification from Multiple Addresses via SMTP Server?

When you don't enable the SMTP Settings for email notifications, you can send each notification from whatever email address you designate in each form's notifications settings: one address for the user, another for the form owner. That's brilliant!!
But when you [do] enable SMTP for notifications, MF defaults to sending all your notifications from all your forms from [1] email address: whatever email you use when you set your SMTP User Name [even if you specify different email addresses, they are [all] overridden by the SMPT username settings]
:.[Is it possible to send notifications from a unique email address for each form using SMTP??

That's actually depend on the policy of the SMTP server. In your case, the SMTP server doesn't seems to allow custom "from email address".
You might want to try using Mandrill for the SMTP server:
http://mandrill.com/They worked great and their free plan allows you to send up to 12.000 emails per month.
