   In setting a custom email, the confirmation email comes across as xyz@xyz.com on behalf of 123@123.com. Is there a way not to have the "on behalf of" and only the custom email itself? If the person hits reply, it ends up going to the "on behalf of address" instead of the custom set email.

   That "on behalf of" is actually coming from the mail transfer agent on your server (or SMTP server if you are using it).
   Some mail transfer agent is configured that way to prevent spamming. You might want to contact your hosting tech support to confirm regarding this.

   Alternatively, configure your MachForm to use SMTP server that allow sending email using any custom "From Email Address".
   Try signup for Mandrill (http://mandrill.com/) -- their free plan allows you to send up to 12k email per month and it works great.
