   http://wufoo.com/ - this looks similar to Machform (although they seem to provided it as a hosted service, not an install-yourself product like Machform.

   This sample on their site is almost identical to the kind of forms Machform produces:

   Yuniar,

   If you get this, YES my email is still valid. Will be keen to do the Beta testing of version 3.

   I tried your "Contact Us" form, I got the "thanks" message but not an auto reply. (EDIT: Scratch that last comment....the reply just turned up, and I see my response is in your help-desk system).
