Adding Features to the Form

    Hello,

    I'm attempting to create something like this:

    Can anyone share any technical insight on how to do something like that with MachForm? For example, would I be able to fit icon images as a field? Any help would be wonderful! Thank you.

    I must say that would require pretty much modifications.
    However, at the basic, you can insert images to any of your field. So you can put images as the selection of a checkbox field or multiple choice field.

    Simply insert the HTML tag of the image to any of your field's label or options (in case of checkboxes or multiple choice)
